Where can I find info on the health occupations test?

Where can I find info on the health occupations test? I'm about to enter school for occupational therapy and I need to take the HOT within the next few months. I've began researching it and I can't find a website that gives me info on the test or test centers around me that carry it. If you have taken it before, where did you find a location and how hard/easy was the test?
